Clinical Services Director

This is a Full-time position in Kent, WA posted September 21, 2021.

The Director of Clinical Services manages, supervises, coordinates, evaluates, and develops client care teams to ensure quality care delivery and appropriate case management within WHAH scope of services and policies; state, federal, and local laws; and scope of Practice for each Discipline.

ESSENTIAL FUNCTIONS

  1. Manages and directs a team of Clinical Case Managers, Therapists, and Home Health Aides; ensuring safe, effective and appropriate Home Health services.
  2. Available at all times during regular business hours and as needed to provide support and assure quality care delivery to Home Health patients.
  3. Receives referrals, determines services required, and Health and Home’s ability to meet needs. Assigns appropriate clinicians to cases in conjunction with Intake Coordinator.
  4. Meets with clinical staff on a regular basis to provide guidance and information related to specific issues. Provides direction to teams to assure that client/patient needs are met and services are provided according to the plan. Assists clinical staff in establishing priorities, setting goals, and evaluating progress toward goals.
  5. Directs case conferences and other clinical meetings to facilitate coordination of care.
  6. Collaborates with the Executive Director in meeting operating budget needs and priorities for the department.
  7. Coordinates 24-hour HH coverage by Registered Nurses. Evaluates quality of on-call services.
  8. Provides on-call backup for clinical staff, as needed.
  9. Develops working relationships with other health care professionals in the community and families to identify resources available and to ensure access of information to clients.
  10. Provides support and direction to HH staff, other health care professionals, clients/patients, and families related to appropriate and available health care resources.
  11. Other duties as assigned (Educates HH Staff on clinical services, provides monthly caseload review to all Case Managers, assists in the screening, interviewing and orientation of new HH personnel, etc.)

QUALIFICATIONS/REQUIREMENTS

Education Background

  1. Graduate of an accredited school of professional nursing.

Licensure, Registration, Certification

  1. Current license as a Registered Nurse in Washington State.
  2. Current and valid Washington State driver’s license and valid vehicle insurance.
  3. Current CPR certification.

Special Training/Skills

  1. Knowledge of Home Health federal and state regulations.
  2. Knowledge of reimbursement sources and documentation requirements within Home Health care.
  3. Excellent verbal and written communication skills and strong interpersonal skills.
  4. Strong computer skills in MS Office suite (Word, Excel, PowerPoint).

Work Experience

  1. Minimum of two (2) years nursing experience in an acute hospital setting or equivalent experience with one year of supervisory experience.

Physical Demands

  1. Sitting at desk at intervals throughout day.
  2. Opening/closing of file drawers.
  3. Data entry into computer at intervals throughout day.
  4. Able to bend, stoop, squat at intervals throughout day.
  5. Good physical and mental health.

INFECTIOUS RISK CATEGORY

HIGH RISK. Likely to have frequent occupational exposure with patient’s blood or other body fluids. May require protective equipment to be worn during some aspects of direct patient care.

SOME (COVID-19) RISK: Possible occupational exposure to residents or clients with COVID-19. May require personal protective equipment (PPE) to be worn during some aspects or seasons of job.
